Most organizations have process documentation. Very few have process documentation anyone trusts. The maps were drawn during an ERP programme three years ago, they describe the intended flow rather than the real one, and nobody has owned them since the consultants left.
That gap stays invisible until you try to automate something. Automation applied to an undocumented process does not fix the process — it hardcodes whatever was happening, including the workarounds, and makes it considerably harder to change later.
The same gap defeats AI. A model asked to reason about your operations can only work from the record you can give it. If the record is a slide deck describing an idealised flow, the answers will describe an idealised company.
